Please be advised, documents submitted through this form are accepted as UNOFFICIAL. Documents, such as college transcript(s), will require the submission of an official transcript(s) from the Institution(s) prior to enrollment.

DO NOT submit grade reports, screenshots of transcripts, transcript formats obtained from a source outside the sending institution or that have been written on or altered in any manner. Transcripts must include front and back of every page including the institution grading page or key.

If you have applied to the university through ApplyUTA and have already submitted your documents please allow time for processing and do not submit them again. Submitting duplicate documents could delay processing. You can login to your student portal to see what documents are still needed.
